Forms
Forms collect structured feedback — surveys, beta sign-ups, feature requests, internal retros — and route every response into the queue, ready to triage.
Forms are a Pro feature. They're available on Pro and Enterprise. Basic collects internal feedback only — upgrade to add forms and widgets.
Building a form
Open Forms from the sidebar and create one from scratch or a template. Add fields, set which are required, and write the intro and confirmation copy. Supported field types include:
- Short and long text
- Single- and multi-select
- Rating and scale
- Email and name (to tie a response to a person)
Public vs. internal forms

Sharing & embedding
Each form gets a shareable link you can send directly, and (for public forms) an embed snippet to drop into a page. Open or close a form to submissions at any time.
Responses in the queue
Submissions flow into the project's feedback queue with the form's name as the source, so you triage them like any other item — set priority, assign an owner, add tags. Responses that include an email can be continued as a support thread.
